What you need to know before completing the Membership Application.

Minimum Qualifications

There are three requirements:

  1. Must be an Ontario resident
  2. Must have employment with Sunnybrook Health Sciences Centre or affiliated organizations or be related to someone who is
  3. Must have a satisfactory credit bureau rating

Provide a photocopy of your Ontario Driver's License with your photograph and a photocopy of one additional piece of identification.

Complete the online membership application and fax the personal identification to:

If you require assistance in completing this application, please call (416) 480-4467.  Our staff will be available to assist you between 8:30 a.m. and 4:30 p.m. Monday to Friday.  You can also leave a voice message at the above number.

Upon receipt of your signature and photocopies of the required personal identification, we will review your membership application for approval.  Approval of the membership application is subject to satisfactory credit check and a membership fee of $65.00 for your member ownership share.  Your membership application will normally be approved within 24 hours of us receiving your completed application and fee.  A representative of the credit union will contact you to confirm details of your application and discuss any questions you may have.
